I believe that pic was included in the booklet that was with the first RCA / BMG CD release of this event.

Marilyn Evans was her name when she was dating Elvis. Today she is Marilyn Knowles-Riehl.

A funny Story from Erik Lorentzen about the mystery girl:
When Jerry Lee Lewis signed the photo in the early seventies, he said: "This is Elvis' girlfriend sittin' on the piano, but she is looking at me."
I told Elvis what the Killer had said, when i met him backstage in Las Vegas in April 1975.
Unconcerned , Elvis replied : "Son, when i date a girl and bring her out, she never looks at another man."
And then he laughed.

At left, we can see Jud Phillips' signature, if I'm not mistaken.

And Perkins' and Cash's signatures appear to come from the same pen and day.
