Like it or loathe it, the stars with the biggest following in the world at the moment are One Direction. Harry Styles has nearly 20 million followers on twitter, and there are very few countries that they have yet to conquer.
The name Clint Eastwood is known throughout the world from all ages and generations regardless if he hasn't been in front of a camera for a few years.

There are other stars who could also claim to be the biggest star in the world by just using one name like Ali, Madonna, Arnold, Stallone.
We're talking 2013 not 1993. That's a bit like saying Crosby was a bigger star in 1958 than Elvis. Conservative society would have liked to have said that was the case, but it wasn't.
Yeah, whatever.

You just want another fight.

I think it's nigh-on immeasurable when it comes to ranking the most famous people in the world today, be it sportsmen, women, stars of music, film or otherwise. More than ever, stars have far more opportunities and avenues to present themselves globally, whether doing so via YouTube, Facebook, Twitter or their own dedicated internet sites. All of which is accessible by fans anyplace, anywhere and anytime. Which means that the biggest and most prolific stars, such as Beyonce or One Direction, can reach people and places less attainable than even twenty years ago, let alone when Elvis, Crosby, Sinatra, Gable, Brando or Monroe were the biggest stars in the world. But there's still international flavours and demographics that don't always translate across the board. David Beckham may be the biggest star globally, but not so much in America. Andy Lau is immensely popular in China, but not so much in the west, ditto A.R. Rahman in India. And although Beyonce, Lady Gaga or Tom Cruise can boast a global popularity that's surpassed by no-one, they mean less than the biggest home-grown stars do in Japan, Hong Kong or the middle east, despite the nature of their fame and popularity. But there's no certain answer here, just opinions. Unless we count what's countable in YouTube hits, friends on Facebook and Twitter followers, although doing so won't paint the entire picture.

None of them names come anywhere near to approaching the star status of Muhammad Ali or Madonna.

Fame is the word we should be using for many of the stars today, and when it really gets down to it it should be 'infamous'. For starters Miley Cyrus has gone from wholesome to downright slutty, all in the name of fame. IMO Miley is the biggest name out there for all the wrong reasons, she comes across as desperate and whoever is advising her has made her sell her soul. Secondly is Justin Bieber, another soul sadly sold to the fame monster. Unfortunately both of these individuals come across as crass, greedy & shallow.

If we are looking from a monetary stand point, then here is the Forbes list
