I know it is on a FTD CD, but when was it recorded and when was it first issued

Elvis' "What's so controversial about me?/Fight Polio" public service announcement for the 1957 March of Dimes was recorded in December 1956. Elvis' PSA was cut #8 on Side 1 on MARCH OF DIMES GALAXY OF STARS (GM-8M-0653/4) a sixteen-inch record featuring 20 celebrities.
Emceed by Howard Miller. The audio project was sponsored by The National Foundation For Infantile Paralysis.
Broadcast window: January 2-31, 1957.
NOTE: There were 2 records distributed to radio stations.
One was the spoken psa platter and the other record (GM-8M-0657) included a song by each of the celebrity vocalists (ie Elvis, Dinah Shore, Eddie Fisher, Pat Boone, Sammy Davis, Jr.)
Elvis' PSA outcue about singing "Love Me Tender" was deliberately there to cue the deejays to play that song which was Cut #2 on Side 1 on the March of Dimes song record. (Most stations had two turntables)
